General Information

Rehabilitation of Offenders Act

Please provide two references, one of which must be from your present or most recent employer. Referees should not be friends, relatives or immediate colleagues. If this is your first employment, one reference must be from your head teacher or lecturer. All appointments are subject to the receipt of two satisfactory references. We reserve the right to take up references from any previous employer.

The information I have provided on this application form is, to the best of my knowledge, correct. I understand that falsifying or withholding information may result in my immediate removal from the PSA register if appointed. I understand that the information given on this application form will be handled and processed in accordance with the Data Protection Act 1998. If I am appointed, the information will form part of my personnel record. I understand that it may be used for statistical analysis and to administer the Company’s operations.
Guidance Notes – Completing the Application Form

Thank you for the interest you have shown in registering as a PSA with Look Ahead Housing and Care.
To assist you in completing the application form please read the following guidance notes.

When completing the application please ensure that:
  • You enter the types of PSA activities you are interested in carrying out,
  • You complete all sections of the form and do not substitute it with a curriculum vitae (CV),
  • You complete the application in black ink or type
  • You sign and date the application form.
Remember, you should:
  • Do a rough draft first so that you avoid mistakes and repetitions on your application form.
  • Check that all sections are completed, including references, employment history and personal details.
  • Keep a photocopy of the application for your own records.
PSA Summaries of General Competencies
The ‘PSA general competencies’ summarise important information about the types of skills, attitudes and behaviours we are looking for. In your supporting statement you should provide us with examples/ evidence of how you meet these criteria and how you believe your competencies, knowledge and experience will benefit our customers. If you do not complete this section fully, with relevant examples, then you are unlikely to be shortlisted.

Criminal Convictions & Cautions
Look Ahead complies fully with the Protection of Vulnerable Adults (POVA) and Criminal Records Bureau (CRB) Codes of Practice and undertakes to treat all applicants fairly. It undertakes not to discriminate unfairly against any subject of a Disclosure on the basis of a conviction or other information revealed.

Where a disclosure is to form part of the recruitment process, we encourage all applicants called for interview to provide details of their criminal record at an early stage in the application process. We request that this information is sent under separate, confidential cover, to a designated person within Look Ahead and we guarantee that this information will only be seen by those who need to see it as part of the recruitment process.

Unless the nature of the position allows Look Ahead to ask questions about your entire criminal record (under the Exemptions Order 1975), we only ask about ‘unspent’ convictions as defined in the Rehabilitation of Offenders Act 1974. As part of the recruitment process to become a PSA with Look Ahead, because you will be working with vulnerable persons, you will be required to undertake POVA and Advanced CRB checks. You will also be required to disclose all criminal convictions, including all those which would otherwise be considered ‘spent’.

We ensure that all those in Look Ahead who are involved in the recruitment process have been suitably trained to identify and assess the relevance and circumstances of convictions and cautions. We also ensure that they have received appropriate guidance and training in the relevant legislation relating to the employment of ex-offenders, e.g. the Rehabilitation of Offenders Act 1974.

At interview, or in a separate discussion, we ensure that an open and measured discussion takes place on the subject of any offences or other matter that might be relevant. Failure to reveal all criminal convictions and cautions could lead to your immediate withdrawal from Look Ahead’s PSA register.

Having a criminal record will not necessarily bar you from working with us.
